“Sitting is the new smoking.”
You have heard it at health conferences, read it in wellness articles, and probably seen it on a poster in a physiotherapy waiting room. The claim is dramatic, memorable, and effective at making people feel guilty about their office job.
It is also misleading.
The idea that sitting itself causes lower back pain has become so widespread that it is rarely questioned. Entire industries have grown around it: standing desks, ergonomic chairs, posture correctors, sit-stand timers, and workplace wellness programs designed to reduce sitting time.
But the relationship between sitting and back pain is far more complex than “sitting is bad.” And misunderstanding it leads people toward solutions that do not work, while ignoring the factors that actually matter.
What the Research Actually Shows
The claim that sitting causes back pain feels intuitive. You sit at a desk for eight hours, your back hurts, so sitting must be the cause. The logic seems airtight.
Except the research does not support it nearly as cleanly as you would expect.
A large systematic review published in the Journal of Biomechanics examined the evidence linking sitting time to lower back pain. The findings were mixed at best. While some studies found a weak association between prolonged sitting and back pain, others found no significant relationship. The overall evidence quality was rated as low, and the reviewers concluded that occupational sitting is not a strong independent risk factor for lower back pain.
A separate review published in the British Journal of Sports Medicine reached a similar conclusion: total sitting time was not consistently associated with back pain across the available evidence base.
This does not mean that sitting has zero effect on your back. It means that sitting alone, as a single isolated factor, is not the clear-cut cause that the wellness industry presents it as. The picture is much more nuanced.
What Actually Causes Lower Back Pain in Desk Workers
If sitting is not the primary villain, what is? The research points to several factors that have stronger and more consistent associations with lower back pain.
Sustained static postures without variation. The problem is not sitting itself. It is sitting in one position without moving for extended periods. The spine is designed for movement. When it is held in any single position for too long (sitting, standing, or anything else), the tissues adapt to that position. Muscles that are held in a shortened position become tight. Muscles that are held in a lengthened position become weak. Intervertebral discs, which rely on movement to circulate nutrients and fluid, become dehydrated and less resilient.
This means that standing all day at a standing desk is not inherently better than sitting all day. It is a different static posture with a different set of problems (increased lower limb fatigue, varicose vein risk, and its own pattern of muscular imbalance). The solution is not standing instead of sitting. It is movement variation throughout the day.
Weak deep core muscles. The muscles that stabilise your spine (transverse abdominis, multifidus, pelvic floor, and diaphragm) are supposed to activate automatically to support your lumbar spine during sitting. In people who are deconditioned or have poor core engagement patterns, these muscles do not fire effectively. The spine loses its active support system and relies instead on passive structures: ligaments, joint capsules, and disc walls. Over hours of sitting, these passive structures accumulate load and begin to ache.
This is one of the most important and least discussed contributors to desk-related back pain. The chair is not the problem. The muscles that should be supporting your spine while you are in the chair are not doing their job.
Poor movement habits outside of work. A desk worker who sits for eight hours and then sits on the couch for four more hours is not just accumulating sitting time. They are accumulating inactivity. The body adapts to what it does most frequently. If you rarely move your spine through its full range of motion, the muscles and connective tissues that enable that range gradually stiffen and weaken.
Conversely, desk workers who exercise regularly, even moderately, have consistently lower rates of lower back pain than those who do not. A meta-analysis in JAMA Internal Medicine found that exercise reduced the risk of developing lower back pain by 33% to 45%, depending on the type and frequency. The protective effect of movement is one of the strongest and most consistent findings in back pain research.
Stress and psychological factors. This is the factor that surprises most people. Research has consistently identified psychosocial factors as strong predictors of chronic lower back pain. Job dissatisfaction, perceived stress, anxiety, depression, and catastrophising (believing that back pain indicates serious damage) are all independently associated with the development and persistence of lower back pain.
A study published in The Lancet described chronic low back pain as a “biopsychosocial” condition, meaning that biological, psychological, and social factors all contribute to the experience of pain. Treating it as a purely mechanical problem (fix the chair, fix the posture, fix the sitting) misses the psychological component entirely.
This does not mean the pain is “in your head.” It means that pain is a complex output of the nervous system, influenced by tissue health, movement patterns, stress levels, sleep quality, beliefs about pain, and emotional state. All of these factors matter.
The Posture Myth
Closely related to the sitting myth is the posture myth: the idea that there is one “correct” sitting posture and that deviating from it causes injury.
Sit up straight. Shoulders back. Feet flat on the floor. Screen at eye level. If you just sat “properly,” your back would not hurt. Right?
Not quite. Research on posture and back pain has consistently failed to identify a single “ideal” posture that prevents pain. A systematic review in the European Spine Journal found no strong evidence that any specific spinal posture is associated with lower back pain. People with “perfect” posture get back pain. People with “terrible” posture live pain-free.
The current consensus in pain science and physiotherapy is that the best posture is your next posture. Variation matters more than position. Moving frequently between positions (sitting, standing, walking, stretching) keeps the tissues loaded dynamically rather than statically, and dynamic loading is what the spine is designed for.
This does not mean posture is irrelevant. It means that the obsessive pursuit of a single “correct” position is misguided. What matters is having the muscular capacity to support multiple positions and the habit of changing position regularly.
What Actually Helps
If sitting is not the primary cause and perfect posture is a myth, what actually reduces back pain for desk workers?
Regular movement throughout the day. Standing up, walking for a few minutes, stretching, changing position. The interval matters more than the activity. A brief movement break every 30 to 45 minutes has been shown to reduce musculoskeletal discomfort more effectively than any ergonomic intervention.
Strengthening the deep core and posterior chain. The muscles that support the lumbar spine need to be strong enough to do their job for the hours you spend sitting. Reformer pilates specifically targets the deep stabilising muscles (transverse abdominis, multifidus) that provide active spinal support. Two to three sessions per week builds the muscular endurance needed to maintain spinal stability through a full workday.
Hip and thoracic mobility. Desk workers commonly develop stiff hips and a rigid thoracic spine (upper back). When these areas lose mobility, the lumbar spine compensates by moving more than it should. Restoring hip and thoracic mobility reduces the compensatory load on the lower back. Reformer exercises like hip circles, mermaid stretches, and thoracic rotation work specifically target these areas.
Stress management. Given the strong association between psychological stress and back pain, managing stress is not a soft add-on. It is a core part of the treatment plan. Exercise itself is one of the most effective stress management strategies available, which creates a positive feedback loop: exercise reduces stress, reduced stress reduces pain sensitivity, reduced pain increases willingness to exercise.
Education about pain. Understanding that back pain does not necessarily mean structural damage, that pain is modifiable, and that movement is safe and beneficial, changes outcomes. Research on pain neuroscience education has shown that simply teaching people about how pain works reduces fear-avoidance behaviour and improves functional outcomes.
The Role of Reformer Pilates for Desk Workers

The Bottom Line
Sitting is not the enemy. Inactivity is. Static posture is. Weak muscles are. Stress is. Lack of movement variety is.
If you work at a desk and your back hurts, the solution is not a better chair (though that can help). The solution is building a body that can tolerate sitting because it is strong, mobile, and well-supported by the muscles that are designed to protect it.
Blaming the chair is easy. Building the strength to not need the chair’s support is harder, but it is the only approach with lasting results.
